Responsibilities

• Support care planning for clients with complex conditions and arrange waiver services.
• Refer clients to community resources to enhance services.
• Document progress notes, advocacy, and care per company policies.
• Work on evaluations for discharge planning, transitions of care, and services.
• Facilitate authorizations and orders for client services.
• Develop relationships and knowledge of community services.
• Facilitate patient access to recommended services.

Qualifications

• Minimum 2 years experience in Home Care or related field.
• Knowledge of Home Care Regulations and timelines.
• Excellent time management and teamwork skills.
• Working knowledge of office equipment.
