Job Description

We are looking for a dedicated Care Manager to join Sunrise Senior Living, where you will have the privilege of providing top-quality care and services to residents in our assisted living and reminiscence neighborhoods. As a Care Manager, you will build meaningful relationships with seniors and their families, assist with daily living activities, and ensure a comfortable and enriching environment for residents.

Responsibilities

• Provide personalized care and assistance to a designated group of residents.
• Monitor and report changes in residents' conditions to management.
• Participate in developing Individualized Service Plans and ensure their implementation.
• Promote a safe and engaging community environment for residents.
• Deliver exceptional dining experiences and uphold service standards.

Qualifications

• Dedication and passion for serving seniors with excellent customer service.
• Ability to work collaboratively in a team-oriented setting.
• High School diploma/GED; certifications may be required.
• Prior experience working with seniors is preferred.
• Strong communication, organizational, and decision-making skills.
