CARE TRANSITIONS SOCIAL WORKER - LBSW/LMSW/LCSW

Job Description

We are looking for a compassionate and dedicated Care Transitions Social Worker (LBSW/LMSW/LCSW) to join the LifeBridge Health team in Baltimore, MD. This full-time position involves working day shifts and collaborating with various healthcare professionals to ensure patient well-being and effective discharge planning.

Responsibilities

• Conduct assessments and facilitate discharge planning for adult and geriatric patients in the Transitions program.
• Collaborate with the Care Transitions nurse to enhance patient outcomes and satisfaction while reducing healthcare costs and readmission rates.
• Perform impromptu home visits to assess safety needs and provide necessary interventions.
• Address psychosocial issues, including cases of abuse/neglect, domestic violence, and mental health concerns.

Requirements

• Bachelor's degree in social work, with LBSW certification.
• Demonstrated experience in social work, particularly in healthcare settings.
