Job Description
We are looking for a dedicated Care Transitions Social Worker with the opportunity to join LifeBridge Health at Sinai Hospital in Baltimore, MD. As a full-time member of our Allied Health team, you will play a crucial role in providing direct services to patients and families, facilitating discharge planning, and ensuring optimal use of healthcare resources.
Responsibilities
- Conduct assessments and develop discharge plans for adult and geriatric patients in the Transitions program.
- Collaborate with Care Transitions nurse to enhance patient outcomes and satisfaction while reducing healthcare costs and readmission rates.
- Perform impromptu home visits to assess safety concerns and provide necessary support.
- Implement psychosocial interventions for cases involving abuse, neglect, domestic violence, and mental health issues.
Minimum Requirements
- Bachelor's degree in social work (LBSW).
- Licensed Social Worker (LBSW).
