As a Care Manager, you will provide top-quality care and services to residents in our assisted living and reminiscence neighborhoods. This role involves building relationships with seniors, assisting with daily activities, monitoring residents' well-being, and contributing to individualized service plans.

Responsibilities

• Build meaningful relationships with seniors and their families.
• Assist residents with daily activities and life skills.
• Participate in the development of individual service plans.
• Ensure residents' well-being and safety throughout the shift.
• Collaborate with the community team to maintain compliance and safety standards.
• Provide dining assistance while upholding hospitality standards.
