Job Description
Sunrise Senior Living is seeking a Care Manager to provide top-quality care and services to residents in our assisted living and reminiscence neighborhoods. As a Care Manager, you will build meaningful relationships with seniors and their families, assist with daily living activities, and ensure individualized care needs are met.
Responsibilities
- Build relationships with residents and families, providing assistance with daily living activities.
- Notify management of changes in condition and recommend appropriate care adjustments.
- Participate in developing Individualized Service Plans (ISP) and monthly updates.
- Ensure residents' safety and well-being during the shift and adhere to safety regulations.
- Respond to residents' dining needs while maintaining hospitality and service standards.
Qualifications
- Dedication to serving seniors with excellent customer service skills.
- Positive attitude and flexibility to perform diverse duties in a team environment.
- High School diploma/GED, with certifications as per state requirements.
- Previous experience working with seniors preferred.
- Strong communication, organizational, and problem-solving skills.
