Job Description
Monadnock Family Services is seeking a Health and Wellness Specialist Case Manager to join our team. In this role, you will provide nutritional assessments, evaluate diet and food intake, digestive issues, and grocery shopping habits to make nutritional recommendations for optimal wellness. Collaborate with psychiatric and medical services as well as the client's treatment team to provide client education around healthy eating and lifestyle choices. This position involves coordinating various therapeutic, educational, and integrative services for psychiatrically disabled children to ensure successful maintenance in the least restrictive environment.
Qualifications
- Bachelor's Degree in a human service field with a degree, certificate, or concentration in health, nutrition, or wellness.
- At least one year of experience working with SED children and their families.
- Skills in advocacy, outreach, assessment, crisis intervention, and supportive counseling.
- Strong communication skills and a good driving record.
Requirements
- Bachelor's Degree in psychology, social work, psychosocial rehabilitation, or related human services field OR Associate's degree in a related field with two years' experience working with individuals with mental illness.
- Understanding of mental illness and a desire to reduce stigma.
- Skills in advocacy, outreach, assessment, crisis intervention, and supportive counseling.
- Strong communication and organization skills.
