Job Description
We are currently seeking a passionate and skilled Complex Care Social Worker - LMSW to join our team at LifeBridge Health. As a full-time member of our allied health team at Sinai Hospital in Baltimore, MD, you will be responsible for facilitating discharge planning for high-cost, difficult-to-place, and escalated patients to meet length of stay targets.
Responsibilities
- Implement a multi-step escalation protocol to identify patients with barriers to discharge.
- Work with the interdisciplinary team to manage escalated patients and adhere to discharge planning standards.
- Monitor high-cost patients for early intervention opportunities to expedite cost-effective discharges.
Requirements
- Master's degree or equivalent seasoned professional knowledge.
- Licensed Master Social Worker (LMSW) certification required.
