Job Description

We are looking for a dedicated Complex Care Social Worker (LMSW) to join our team at LifeBridge Health. As the Complex Care Social Worker, you will play a crucial role in facilitating discharge planning for challenging cases, ensuring efficient and cost-effective care delivery.

Responsibilities

• Implement a multi-step escalation protocol to identify patients with barriers to discharge.
• Work collaboratively with the interdisciplinary team to address patient needs and achieve timely discharges.
• Monitor difficult to place and high-cost patients for early intervention opportunities.
• Adhere to set standards for discharge planning and utilization review guidelines.

Requirements

• Master's degree or equivalent professional knowledge.
• Licensed Master Social Worker (LMSW) certification.
