Job Description
We are looking for a dedicated Complex Care Social Worker - LMSW to join our team at LifeBridge Health. This full-time position, located in Baltimore, MD at Sinai Hospital, offers a day shift schedule from 8:00am to 4:30pm with a sign-on bonus potential of $15,000. As a Complex Care Social Worker, you will play a crucial role in facilitating discharge planning for challenging cases involving difficult to place, high-cost, and escalated patients.
Responsibilities
- Execute a multi-step escalation protocol to identify barriers hindering effective discharges.
- Work collaboratively with the interdisciplinary team to manage escalated cases and meet discharge planning standards.
- Monitor challenging cases for early intervention opportunities to ensure timely and cost-effective discharges.
Requirements
- Master's degree or equivalent professional knowledge.
- Licensed Master Social Worker (LMSW) certification.
