Job Description

Westminster Canterbury of Lynchburg, a 5-Star rated Continuing Care Retirement Community in Central VA, is looking for a dedicated Healthcare Admissions Coordinator to join our team. Responsibilities

  • Manage health services lead information and coordinate admissions to both Assisted Living and Healthcare.
  • Act as a liaison between WC and community referral sources.
  • Promote resident admissions to WC health care services by calling on physicians, hospitals, and other relevant sources.
  • Notify departments about new resident move-in dates and locations.
  • Perform social work functions for skilled nursing/rehab residents, including comprehensive program management and discharge planning.
  • Support residents and families during their rehab stay, addressing psycho-social needs and contributing to treatment plans.

Requirements

  • Bachelor's Degree in a human services field.
  • Minimum of 5 years' healthcare experience.
