Liberty Home Care is seeking a dedicated HOME CARE PHYSICAL THERAPIST (PT) to join our team. In this role, you will provide patient care services, develop personalized care plans, and ensure the well-being of our patients in the comfort of their homes.

Responsibilities

• Provide direct patient treatment and follow-up services.
• Evaluate patients, create tailored care plans, and document progress accurately.

Requirements

• Bachelor's degree in physical therapy and current state licensure.
• Minimum one year of therapeutic experience.
