HOME CARE PHYSICAL THERAPIST (PT)

Job Description

Join Liberty Healthcare Management as a Home Care Physical Therapist (PT) and make a difference in improving patient outcomes. In this role, you will provide direct patient care services, evaluate new patients, develop care plans, document patient data accurately, communicate with various parties, and coordinate referrals to appropriate disciplines. Additionally, you will provide education, supervise peers, support personnel, and assist in research and orientation activities.

Requirements

• Bachelor's degree in physical therapy and current state licensure.
• Minimum of one year's experience in therapeutic practice.
• Strong knowledge of physical therapy theory and concepts.
• Ongoing continuing education.
• Willingness to travel and adapt to different working conditions.
