At Interim HealthCare of West Palm Beach, we are seeking a compassionate and dedicated Home Health Registered Nurse (RN) to join our team in Vero Beach, Ft. Pierce, and Port St. Lucie, Florida. Responsibilities:

• Provide home-based care to patients with illnesses, injuries, and chronic diseases.
• Collaborate with physicians and oversee care provided by LPNs, CNAs, and aides.
• Conduct patient assessments, document progress, and communicate changes to physicians.
• Educate patients and families on care plans, medications, therapy, and safety.
• Ensure care goals are met and coordinate patient discharge.

Requirements:

• Minimum of 1 year of acute care experience, home healthcare experience preferred.
• Graduate of an accredited nursing program with an active RN license in FL.
• CPR certification and knowledge of state and federal home health regulations.
• Clinical proficiency, strong communication skills, and compassion.
