Home Health Registered Nurse (RN)

Responsibilities

• Deliver home-based care to patients recovering from illnesses, injuries, and chronic conditions.
• Collaborate with a multidisciplinary team and oversee patient care provided by LPNs, CNAs, and aides.
• Conduct patient assessments, document progress, and communicate changes to the medical team.
• Educate patients and their families on treatment plans, medications, and health maintenance.
• Ensure treatment goals are met and facilitate patient discharge when appropriate.

Requirements

• Minimum of 1 year acute care experience, preferably in home healthcare.
• Graduate of an accredited nursing program with an active RN license in FL.
• CPR certification and knowledge of state and federal home health regulations.
• Strong clinical skills, effective communication, and a compassionate approach to patient care.
