Home Health Registered Nurse (RN)

Job Description

We are looking for a compassionate Home Health Registered Nurse (RN) to join Interim HealthCare of West Palm Beach in Vero Beach, Ft. Pierce, and Port St. Lucie, Florida. Responsibilities

• Deliver home-based care to patients with various medical needs.
• Collaborate with a multidisciplinary team to ensure comprehensive patient care.
• Conduct assessments, document patient progress, and communicate changes to physicians.
• Educate patients and families on treatment plans, medications, and wellness strategies.
• Ensure patients' goals are achieved and manage discharge planning.

Requirements

• Minimum 1 year of acute care experience; home healthcare experience preferred.
• Graduate of an accredited nursing program with an active RN license in FL.
• CPR certification and familiarity with state and federal home health regulations.
• Strong communication skills, clinical proficiency, and compassion for patient care.
