We are hiring a Home Health Registered Nurse (RN) to join Interim HealthCare of West Palm Beach. Responsibilities

• Provide home-based care to patients with illnesses, injuries, and chronic diseases.
• Work with a team of physicians and oversee other healthcare providers.
• Conduct patient assessments, document progress, and communicate changes to physicians.
• Educate patients and families on care plans, medications, and safety measures.
• Coordinate discharge and ensure goals are met.

Requirements

• Minimum of 1 year of acute care experience.
• Home healthcare experience and Oasis experience preferred.
• Graduate of an accredited nursing program and active RN license in FL.
• CPR certification required.
• Knowledge of state and federal home health regulations.
• Strong communication skills and clinical proficiency in nursing specialty.
