Home Health Registered Nurse (RN) - Per Diem

Job Description

We are looking for a dedicated Home Health Registered Nurse (RN) to join our team at Interim HealthCare of West Palm Beach. As a part of our organization, you will have the opportunity to provide exceptional patient care in a flexible and autonomous work environment.

Responsibilities

• Deliver home-based care to patients post-hospitalization or rehabilitation, ensuring their wellbeing.
• Collaborate with a multidisciplinary team to create and implement patient care plans.
• Educate patients and their families on treatment plans, medications, and home safety practices.
• Monitor patient progress, document changes, and communicate effectively with physicians.
• Coordinate discharge planning to meet patient goals and ensure continuity of care.

Requirements

• Minimum of 1 year of acute care experience, with a preference for home healthcare experience.
• Active RN license in FL and CPR certification.
• Proficiency in Oasis and knowledge of state and federal home health regulations.
• Excellent clinical skills, strong communication abilities, and compassion for patient care.
