Job Description
We are looking for a motivated Home Health Registered Nurse (RN) to join our team at Interim HealthCare of West Palm Beach. Responsibilities
- Provide home-based care to patients with various health conditions.
- Collaborate with a multidisciplinary team to ensure comprehensive care.
- Conduct patient assessments, document progress, and communicate with physicians.
- Educate patients and families on treatment plans and health management.
- Coordinate discharge planning and ensure goals are met.
Requirements
- Minimum 1 year of acute care experience.
- Active RN license in FL and CPR certification.
- Familiarity with Oasis and state/federal home health regulations.
- Strong clinical skills, communication abilities, and empathy.
