Job Description: Home Health Registered Nurse (RN)

Interim HealthCare of West Palm Beach is seeking a compassionate and experienced Home Health Registered Nurse (RN) to join our team on a Per Diem basis in Vero Beach, Port St. Lucie, and Ft. Pierce areas. In this role, you will provide exceptional patient care while enjoying the flexibility and autonomy that comes with home healthcare.

Responsibilities:

• Provide home-based care to patients with illnesses, injuries, and chronic diseases.
• Work with a team of physicians and oversee other healthcare professionals.
• Conduct patient assessments, document progress, and report changes to physicians.
• Educate patients and families on care plans, medication, therapy, diet, and exercise.
• Ensure care goals are met and coordinate discharge.

Requirements:

• 1 year of acute care experience required, home healthcare experience preferred.
• Graduate of an accredited nursing program with an active RN license in FL.
• CPR certification and knowledge of state and federal home health regulations.
• Clinical proficiency, strong communication skills, and compassion.
