Responsibilities
- Provide home-based care to patients recovering from illnesses, injuries, and chronic conditions.
- Collaborate with physicians and supervise a team of healthcare professionals.
- Conduct patient assessments, document progress, and communicate changes to the medical team.
- Educate patients and their families on treatment plans, medications, and home care routines.
- Ensure care goals are met and facilitate smooth patient discharges.
Requirements
- Minimum of 1 year of acute care experience.
- Home healthcare and Oasis experience are preferred.
- Graduate of an accredited nursing program with an active RN license in FL.
- CPR certification and knowledge of state and federal home health regulations.
- Strong clinical skills, effective communication, and a compassionate demeanor.
