Home Health Registered Nurse (RN) - Per Diem
Interim HealthCare of West Palm Beach is seeking a dedicated Home Health RN to join our team in Port St. Lucie and Okeechobee, FL. As a Home Health RN, you will have the flexibility to create your own schedule and balance your work and personal life effectively.
Responsibilities
- Provide home-based care to patients with various medical conditions
- Collaborate with a multidisciplinary team to ensure comprehensive patient care
- Conduct patient assessments, document progress, and communicate changes to physicians
- Educate patients and their families on treatment plans and health management
- Ensure patients' goals are met and facilitate discharge planning
Requirements
- Minimum 1 year of acute care experience, home healthcare experience preferred
- Active RN license in FL and CPR certification
- Knowledge of state and federal home health regulations
- Strong clinical skills, communication abilities, and empathy
