Job Description
We are currently seeking a skilled Home Health Registered Nurse (RN) on a Per Diem basis to join Interim HealthCare of West Palm Beach. Responsibilities
- Provide home-based care to patients post-hospital discharge.
- Collaborate with a multidisciplinary team to ensure comprehensive patient care.
- Conduct patient assessments, document progress, and communicate changes to physicians.
- Educate patients and their families on treatment plans, medication, and home care.
- Ensure treatment goals are achieved and facilitate patient discharge as necessary.
Requirements
- Minimum of 1-year acute care experience; home healthcare experience and Oasis experience preferred.
- Graduate of an accredited nursing program with an active RN license in FL.
- CPR certification and knowledge of state and federal home health regulations.
- Strong clinical proficiency, communication skills, and compassion.
