Interim HealthCare of West Palm Beach is seeking a passionate Home Health Registered Nurse (RN) to join our team on a Per Diem basis in Port St. Lucie, Fort Pierce, and Okeechobee, FL. Responsibilities

• Provide home-based care to patients post-hospitalization, rehab, or nursing facility discharge.
• Collaborate with physicians and supervise LPNs, CNAs, and aides in patient care.
• Conduct patient assessments, monitor progress, and communicate changes to physicians.
• Educate patients and families on care plans, medications, and health strategies.
• Ensure care goals are met and facilitate smooth discharge planning.

Requirements

• 1 year of acute care experience, preferably in home healthcare.
• Oasis experience is a plus.
• Graduate of an accredited nursing program with an active RN license in FL.
• CPR certification and proficiency in state and federal home health regulations.
• Strong clinical skills, communication abilities, and empathy.
