Job Description
We are seeking a dedicated Home Health Registered Nurse (RN) to join our team at Interim HealthCare of West Palm Beach. As a PRN/Per Diem nurse, you will have the flexibility to create your schedule and provide exceptional care to patients in Vero Beach, Port St. Lucie, and Ft. Pierce areas. Our ideal candidate is compassionate, knowledgeable, and ready to make a difference in the lives of those in need.
Responsibilities
- Provide home-based care to patients post-hospital discharge, rehabilitation, or nursing facility stay.
- Collaborate with physicians and lead a care team to ensure optimal patient outcomes.
- Conduct patient assessments, document progress, and communicate changes to the medical team.
- Educate patients and families on treatment plans, medications, and overall wellness.
- Coordinate care plans, monitor progress, and facilitate smooth discharges.
Requirements
- Minimum 1 year of acute care experience; home healthcare experience is a plus.
- Graduate of an accredited nursing program with an active RN license in Florida.
- CPR certification and proficiency in Oasis documentation preferred.
- Strong clinical skills, communication abilities, and knowledge of state and federal home health regulations.
