Job Description
We are seeking a Home Health Registered Nurse (RN) to join our team at Interim HealthCare of West Palm Beach. Responsibilities
- Provide home-based care to patients with illnesses, injuries, and chronic diseases.
- Work in collaboration with a team of physicians and oversee other healthcare professionals.
- Perform patient assessments, document progress, and report changes to the physician.
- Educate patients and their families on treatment plans, medications, and home safety.
- Ensure goals are met and coordinate discharge from services.
Requirements
- Minimum of 1 year of acute care experience; home healthcare experience preferred.
- Graduate of an accredited nursing program with an active RN license in FL.
- CPR certification required.
- Knowledge of state and federal home health regulations.
- Clinical proficiency in nursing specialty, strong communication skills, and compassion.
