Job Description
We are currently seeking a compassionate and dedicated Hospice Social Worker to join our team at LifeBridge Health. This part-time position offers weekend work only, providing essential services to individuals and families in need. As a Hospice Social Worker, you will play a crucial role in assessing psychosocial, environmental, and financial needs, delivering counseling services, and coordinating community referrals to ensure comprehensive care.
Responsibilities
- Provide professional and compassionate care in accordance with the mission and values of Carroll Hospice and Carroll Hospital Center.
- Assess and address the psychosocial, environmental, and financial needs of patients and families.
- Formulate, implement, and evaluate a personalized care plan in collaboration with patients, families, and caregivers.
- Offer counseling and support throughout the dying and grieving process.
- Coordinate homecare referrals with community agencies and services.
Requirements
- Master's Degree in Social Work from an accredited school.
- Minimum of three years of clinical experience, with experience in a community health setting preferred.
- LCSW or LCPC certification.
- Excellent interpersonal and communication skills.
- Professional licensure in Maryland and Pennsylvania.
