INPATIENT CASE MANAGER (RN)

Job Description

We are looking for an experienced Inpatient Case Manager (RN) to join our team at LifeBridge Health. As the Inpatient Care Manager, you will work in collaboration with the clinical team and medical providers to ensure optimal discharge planning coordination and interventions for patients with complex health needs. Your role will focus on promoting patient wellness, enhancing care outcomes, optimizing health service utilization, and reducing payment denials.

Responsibilities

• Coordinate discharge planning in collaboration with the clinical team and medical providers.
• Promote patient wellness and optimize care outcomes.
• Ensure efficient utilization of health services.
• Minimize denials of payment.

Requirements

• Bachelor's degree or equivalent professional knowledge.
• BSN preferred; ADN required.
• Maryland Registered Nurse License.
• Case Management certification required within 3-5 years of hire.
