INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. As a key member of the care management department, you will collaborate with the clinical team and medical providers to coordinate discharge planning and interventions for patients with complex health needs. The primary goal is to enhance patient wellness, improve care outcomes, optimize healthcare utilization, and reduce denials of payment.

Responsibilities

• Collaborate with the clinical team and medical provider to develop and implement discharge plans.
• Promote patient wellness and efficient health service utilization.
• Work towards minimizing denials of payment among a patient population with complex health needs.

Requirements

• Bachelor's degree in Nursing (BSN preferred, ADN required).
• 3-5 years of related experience in a healthcare setting.
• Strong understanding of discharge planning and care coordination.
• Excellent communication and interpersonal skills.
