INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. This full-time position, based in Randallstown, MD at Northwest Hospital, involves collaborating with the clinical team and medical providers to coordinate discharge planning and interventions for patients with complex health needs. The role focuses on promoting patient wellness, improving care outcomes, and ensuring efficient healthcare service utilization.

Responsibilities

• Coordinate discharge planning in collaboration with clinical team and medical providers.
• Promote patient wellness and improved care outcomes.
• Ensure efficient utilization of health services and minimize payment denials.

Requirements

• Bachelor's degree in nursing (BSN) from CCNE accredited schools.
• Basic professional knowledge in a specialized field.
• Prior experience as a Utilization Review Nurse, Clinical Care Coordinator, Patient Care Integrator, or RN Case Manager at LifeBridge Health (LBH) is preferred.
