INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. As an Inpatient Care Manager, you will collaborate with the clinical team and medical provider to coordinate discharge planning and interventions for patients with complex health needs, ensuring improved care outcomes and efficient health service utilization.

Responsibilities

• Provide discharge planning coordination and intervention.
• Promote patient wellness and improved care outcomes.
• Ensure efficient utilization of health services.
• Minimize denials of payment among patients with complex health needs.

Requirements

• Bachelor's degree in a related field; BSN preferred, ADN required.
• 3-5 years of related experience.
• Maryland Registered Nurse License (Intent to achieve MD licensure if out of state).
• Case Management certification required within 3-5 years of hire.
