INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. As an Inpatient Case Manager, you will work closely with the clinical team and medical providers to coordinate discharge planning and interventions for patients with complex health needs. Your role will focus on promoting patient wellness, improving care outcomes, and ensuring efficient utilization of health services.

Responsibilities

• Collaborate with the clinical team and medical providers to facilitate discharge planning.
• Promote patient wellness and improved care outcomes.
• Ensure efficient utilization of health services.
• Minimize denials of payment among patients.

Requirements

• Bachelor's degree in Nursing (BSN) from CCNE accredited schools.
• Professional knowledge within a specialized field.
• Prior experience in Utilization Review, Clinical Care Coordination, or Case Management is a plus.
