INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. As the Inpatient Care Manager, you will collaborate with the clinical team and medical providers to coordinate discharge planning and interventions. Your role is crucial in promoting patient wellness, improving care outcomes, optimizing health service utilization, and reducing payment denials for patients with complex health needs.

Responsibilities

• Collaborate with healthcare professionals and patients to develop effective discharge plans.
• Promote patient wellness and ensure efficient utilization of health services.
• Facilitate communication between patients, families, and healthcare providers.

Requirements

• BSN preferred; ADN required.
• Maryland Registered Nurse License.
• Case Management certification required within 3-5 years of hire.
