Job Description
We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. As a full-time member, you will work closely with the clinical team and medical provider to coordinate discharge planning and interventions for patients with complex health needs. Requirements
- Basic professional knowledge with a Bachelor's degree; specialized field expertise.
