INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated and experienced Inpatient Case Manager (RN) to join our team at LifeBridge Health. The Inpatient Case Manager will work collaboratively with the clinical team and medical providers to coordinate discharge planning and interventions for patients with complex health needs. The primary focus will be on promoting patient wellness, improving care outcomes, optimizing health service utilization, and reducing payment denials.

Responsibilities

• Coordinate discharge planning with the clinical team and medical providers.
• Promote patient wellness and ensure improved care outcomes.
• Efficiently utilize health services and minimize payment denials.
• Provide case management services in compliance with regulations and standards.

Requirements

• Bachelor's degree in a related field; BSN preferred.
• Active Maryland Registered Nurse License.
• Case Management certification required within 3-5 years of hire.
• Knowledge and experience in the healthcare industry.
