INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. As an Inpatient Case Manager, you will collaborate with the clinical team and medical provider to coordinate discharge planning and interventions for patients with complex health needs. Your role will focus on promoting patient wellness, improving care outcomes, ensuring efficient utilization of health services, and reducing denial of payments.

Requirements

• Bachelor's degree or equivalent professional knowledge; BSN preferred, ADN required.
• 3-5 years of related experience in a healthcare setting.
