INPATIENT CASE MANAGER (RN) - PART TIME

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. This part-time role based in Randallstown, MD at Northwest Hospital involves discharge planning coordination, intervention, and promoting patient wellness. As a key member of the clinical team, you will work closely with medical providers to ensure efficient utilization of health services and improve care outcomes for patients with complex health needs.

Responsibilities

• Collaborate with the clinical team and medical provider for discharge planning coordination.
• Promote patient wellness and improved care outcomes.
• Ensure efficient utilization of health services.
• Minimize denials of payment among patients with complex health needs.

Requirements

• Bachelor's degree or equivalent professional knowledge.
• BSN required from CCNE accredited schools.
• Prior experience in Utilization Review, Clinical Care Coordination, or RN Case Management is advantageous.
