INPATIENT CASE MANAGER (RN) - PRN

Job Description

We are looking for an Inpatient Case Manager (RN) to join our team at LifeBridge Health. As the Inpatient Care Manager, you will collaborate with the clinical team and medical providers to provide discharge planning coordination and intervention for patients with complex health needs. Your role will focus on promoting patient wellness, improving care outcomes, and ensuring efficient utilization of health services.

Responsibilities

• Coordinate discharge planning in collaboration with the clinical team and medical provider.
• Promote patient wellness and improve care outcomes for individuals with complex health needs.
• Ensure efficient utilization of health services and minimize payment denials.

Requirements

• Bachelor's degree in Nursing preferred.
• 3-5 years of relevant experience in case management.
• American Heart Association CPR Certification.
• Maryland Registered Nurse License.
• Case Management certification required within 3-5 years of hire.
