Job Description
We are looking for an Inpatient Case Manager - Social Worker to join our team at LifeBridge Health. In this full-time role based in Baltimore, MD at Sinai Hospital, you will be working in Care Management with a weekend commitment during day shifts. The position offers a Sign On Bonus Potential of $15,000.
Responsibilities
- Provide patient and family advocacy.
- Coordinate discharge planning.
- Offer psychosocial intervention for high-risk inpatients.
- Promote patient and family wellness.
- Ensure access to appropriate hospital and community resources for patients with complex health needs.
Requirements
- Master's in Social Work required.
- 2+ years of hospital social work/community social services agencies experience preferred.
- LMSW required; LCSW/LCSW-C preferred.
- MD Social Work License per level of education.
- Ability to follow verbal instructions and communicate effectively.
