INPATIENT CASE MANAGER - SOCIAL WORKER (LMSW / LCSW-C)

Job Description

We are looking for an experienced Inpatient Case Manager - Social Worker (LMSW/LCSW-C) to join our healthcare team at LifeBridge Health. This full-time position based in Baltimore, MD at Sinai Hospital involves providing patient advocacy, discharge planning coordination, and psychosocial interventions for high-risk inpatients. The ideal candidate will promote patient and family wellness, enhance care outcomes, and facilitate access to hospital and community resources for a patient population with complex health needs.

Responsibilities

• Collaborate with clinical team and medical provider to advocate for patients and families.
• Coordinate discharge planning and ensure access to necessary resources.
• Provide psychosocial interventions for inpatients with complex health needs.

Requirements

• Master's degree in Social Work.
• LMSW required; LCSW/LCSW-C preferred.
• 2+ years of hospital social work/community social services experience.
• Maryland Social Work License.
• Strong verbal and written communication skills.
