Job Description

We are looking for a dedicated Inpatient Case Manager - Social Worker (LMSW) to join our team at LifeBridge Health. In this role, you will provide leadership in coordinating post-acute care services, offering psychosocial support to patients and families, and guiding the healthcare team in optimizing acute care services utilization.

Responsibilities

• Lead the healthcare team in post-acute care coordination.
• Provide psychosocial support services to patients and families.
• Advise on the appropriate use of acute care services and length of stay.
• Participate in departmental operations and improvement projects.

Requirements

• Master's degree in Social Work.
• LMSW license in Maryland.
• 3-5 years of related experience.
