INPATIENT CASE MANAGER - SOCIAL WORKER (LMSW) - PRN

Job Description

We are looking for an Inpatient Case Manager - Social Worker (LMSW) to join our team at LifeBridge Health. In this role, you will provide leadership to the health care team, coordinate post-acute care services, offer psychosocial support to patients and families, and assist in departmental operations and improvement projects. This position is based in Baltimore, MD at Sinai Hospital, offering PRN day shift from 8:00 am to 4:30 pm.

Responsibilities

• Lead the health care team in coordinating post-acute care services.
• Provide psychosocial support to patients and families.
• Act as a resource for appropriate and timely utilization of acute care level services.
• Assist in departmental operations and improvement projects.

Requirements

• Master's degree in Social Work.
• LMSW license in Maryland.
• 3-5 years of related experience.
