INPATIENT SOCIAL WORKER LMSW

Job Description

We are looking for a dedicated Inpatient Social Worker LMSW to join our team at LifeBridge Health. As an Inpatient Social Worker, you will collaborate with the clinical team and medical providers to offer patient and family advocacy, discharge planning coordination, and psychosocial intervention for high-risk inpatients. Your role will focus on promoting patient and family wellness, enhancing care outcomes, and connecting patients with relevant hospital and community resources.

Responsibilities

• Provide patient and family advocacy.
• Coordinate discharge planning.
• Offer psychosocial intervention for high-risk inpatients.

• Connect patients with appropriate hospital and community resources.

Requirements

• Master's degree in Social Work.
• LMSW required; LCSW/LCSW-C preferred.
• 3-5 years of experience in the field.
• Maryland Social Work License.
• Strong verbal and written communication skills.
