Job Description

We are seeking an enthusiastic Intake Assessor to join our dynamic team at The Crossroads Center. As an Intake Assessor, you will play a crucial role in conducting psychosocial assessments, determining appropriate levels of care, and facilitating referrals to specialized programs. Your responsibilities will involve overseeing the intake process for individuals seeking services, ensuring accurate documentation, and providing exceptional customer service to stakeholders.

Key Responsibilities

• Conduct psychosocial assessments and determine suitable levels of care.
• Collaborate with clinical staff to address clinical challenges.
• Maintain accurate documentation in compliance with agency standards.
• Provide exceptional customer service and resolve service issues effectively.

Experience & Qualifications

• Bachelor's degree in social work, counseling, or related field; Master's degree preferred.
• Minimum 2 years of experience in intake and assessment for individuals with substance use and mental health disorders.
• Licensed in the State of Ohio as an LSW, LISW, LPC, or LPCC.
