LPN MDS Coordinator

Job Description

We are currently looking for an LPN MDS Coordinator to join Genesis Healthcare, a leading provider of healthcare services offering a range of care options and clinical services. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs), ensuring quality patient care and appropriate reimbursement for nursing center services.

Position Highlights

• Lead the nursing center interdisciplinary Resident Assessment Instrument (RAI) process in compliance with guidelines for MDS completion, encoding, and electronic State transmission.
• Contribute to the development of personalized resident Care Plans, identifying resident needs and anticipated outcomes as mandated by Federal and State regulations.
• Provide training to new interdisciplinary team members on clinical content and electronic MDS item completion.
