Job Description
Join a compassionate team at Liberty Healthcare & Rehabilitation Services as a full-time MDS Coordinator. This role requires a Registered Nurse with exceptional organizational skills to manage MDS completion, accuracy, and compliance with state guidelines.
Responsibilities
- Maintain and track MDS due dates.
- Coordinate MDS completion across disciplines.
- Verify accuracy of MDS data and make corrections as needed.
- Input completed MDS into the system.
- Ensure timely transmission and approval of assessments by the State.
- Prepare and resubmit any rejected records.
- Create Resident Care Plans in collaboration with the care team.
- Conduct RCP meetings regularly and communicate plans effectively.
- Collaborate with departments to ensure compliance with RCP.
- Perform other duties as assigned by the Director of Nursing or Administrator.
Requirements
- Registered Nurse with an active RN license in North Carolina.
- Familiarity with personnel, department, and facility policies.
- Experience with MDS-RAP and Care Planning functions.
- Preferably knowledgeable about RUG-IV and MDS 3.0.
