Nurse Practitioner NP - Bronx, New York

Job Description

We are looking for a dedicated Nurse Practitioner (NP) to join our Advantmed provider network in Bronx, New York. As an NP, you will be conducting in-home wellness risk adjustment assessments for the Medicare population. Your role will involve assessing the overall health and well-being of Medicare beneficiaries to ensure accurate risk adjustment coding, ultimately enhancing value-based care delivery.

NP Responsibilities:

• Perform annual wellness visits and health assessments for individuals with chronic conditions.
• Provide patient health education opportunities.
• Assist in closing quality care gaps by conducting screenings and labs.
• Collaborate effectively to enhance care delivery.

NP Qualifications:

• Must have a valid unencumbered NP License for New York.
• Prior experience in in-home risk assessments is beneficial.
• Minimum of 3 years of patient care experience required, preferably in primary care/adult/geriatric settings with EMR skills.
• Bilingual proficiency is advantageous.
