Job Description
We are looking for a dedicated Nurse Practitioner (NP) to join OnSite Healthcare and Wellness, a leading healthcare organization founded by nurse practitioners. In this role, you will provide high-level medical and mental health care to patients in various settings, including Assisted Living, Independent Living, Skilled Nursing Facilities, and homes.
Responsibilities
- Round at assigned facilities to manage acute, subacute, and primary care needs of the elderly.
- Collaborate with the OnSite team, nursing home staff, and families to provide comprehensive care.
- Proactively follow up with patients to ensure effectiveness of interventions.
- Provide education on best practices and integrate non-pharmacological treatments.
Requirements
- Master of Science in Nursing with relevant certification.
- Active License in South Carolina and DEA registration.
- Minimum 1 year of experience.
- Strong communication, documentation, and patient assessment skills.
- Ability to work autonomously with a patient-centric approach.

$100,000 - $110,000 yearly salary with performance-based bonuses.
