Nurse Practitioner or Physician Assistant

Job Description

Curana Health, a provider-led healthcare organization, is seeking a Nurse Practitioner or Physician Assistant to join our team dedicated to improving healthcare delivery in senior living communities. As a facility-based healthcare provider, you will deliver direct patient care services in a post-acute care setting, focusing on enhancing the well-being of senior residents.

Responsibilities

• Administer direct patient care visits and re-evaluations.
• Evaluate and treat chronic and co-morbid illnesses.
• Review medications, orders, and diagnostic tests.
• Coordinate care with collaborating physicians and other healthcare disciplines.
• Conduct Annual Wellness Visits and History and Physicals as required.
• Communicate with families and attend conferences when necessary.

Qualifications

• Valid nurse practitioner or physician assistant license in the state of operations.
• Current board certification in the relevant field.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills.
• Excellent written and oral communication skills.
• Ability to work effectively in a team environment.
