Join Liberty Healthcare Management as a Home Care Physical Therapist (PT) on a per diem basis. As a vital member of our team, you will provide patient care services, develop personalized treatment plans, and ensure accurate documentation of patient progress. Additionally, you will collaborate with interdisciplinary teams, educate healthcare professionals, and contribute to research efforts.

Responsibilities

• Provide direct patient treatment and follow-up care.
• Evaluate new patients, create tailored care plans, and document progress.
• Communicate with internal and external contacts regarding patient care and treatment plans.
• Coordinate referrals to other disciplines and support peer education.
• Conduct research and assist in new employee orientation.

Requirements

• Bachelor's degree in physical therapy and state licensure.
• Minimum one year of therapeutic experience preferred.
• Strong knowledge of physical therapy theory and practice.
